The purpose of this questionnaire is to provide a theoretical framework when speaking about leadership in your organization during our upcoming workshop.

There are 10 questions in total. Please note that each question consists of two components.

• Current: Questions in this column allow you to evaluate how you "currently perceive" the behavior of leaders within your company.
• Desired: Questions in this column allow you to evaluate what "you would like to see" the leadership culture evolve towards in future.

Please distribute 10 points across all 3 answers per column, by assigning more points to the statements that fit the question asked. (ex. 6,3,1 or 5,5,0 or 5,3,2..etc)

To help you answer, think about the leaders you work with (peers), leaders you report into (both dotted and straight lines) and the senior leadership of your company (C-Level Global).

Once you receive your results, please take a screen shot. This will be for YOUR reference ONLY! During the workshop, we will only share the average scores of ALL responses.
